I have an interesting problem and I'm not 100% what's causing it.
Setup:
Windows 7 (64-bit) on a Samsung laptop
Firefox browser
Microsoft Office 2010
Classic Shell v3.6.5
Microsoft Wireless Mobile Mouse 3000 (Model 1359)

I have my mouse setup as a left handed mouse. Starting this week sometime, it works fine in everything except Excel. In Excel, it's now a right handed mouse. Just moving the cursor across the Excel window, never even activating it, causes the mouse to reverse to right hand. But clicking on anything else eventually returns the mouse to left hand. (it takes a couple clicks.) I'm posting this here because I've noticed that now my Classic Shell Start Menu is missing. The program is still loaded, I've seen it listed in the Program and Features list and Task Manager says it's running. But if I click on the Start bubble, I get the Windows 7 menu, not my customized Classic Shell menu, and the left side is completely blanked out except for the "All Programs" link. According to Microsoft, after searching the web, the mouse driver is up-to-date. (that's assuming you believe anything Microsoft says.) Anybody seen anything like this or got any suggestions? While usable, it's taxing my brain to remember which button to use to click where. (not to mention I'd like my menu back.)

Classic Shell doesn't do anything to change mouse behavior inside other applications. That is unrelated. Check in All Programs -> Start Menu Settings -> Controls tab (All Settings) or "Basic Settings" tab to see if "Left Click Opens" is set to "Classic Start Menu", not "Windows Start Menu". The left column in your Windows Start Menu might be empty because the Most Frequently Used program history is off. Turn it on by right clicking the taskbar -> Properties -> Start Menu tab -> Check "Store recently opened programs".
