For the Windows 7 style of the Classic Start Menu, Downloads is there already on the Customize Start Menu tab in settings.
If you are using the Classic style of the menu, also go to the Customize Start Menu tab and scroll all the way down in the right column. Drag and drop the "Custom" item to the left column and position it via dragging to where you want it. Now double click the Custom item to show the Edit Menu Item dialog. In the Link: field, type: shell:Downloads Click OK to save settings.
