Classic Shell 3.6.7
Windows 8

Here is the issue:

It appears when this checked:

[General Behavior]
+Pre-cache icons

When the setting is unchecked, the issue disapperas:

[General Behavior]
-Pre-cache icons

And when I check "Pre-cache icons" again then the problem comes back!

I guess there's something wrong with cache.. so is there any way to flush the cache?

The setting only controls if the start menu will go looking for icons before they are needed. The icons are not saved on disk.
What happens if you check the option, right-click on the start button, select Exit, then start ClassicStartMenu.exe again?
I am trying to figure out if the problem is related to loading the icons too soon after login.

1) i uncheck "pre cache icons"
2) i exit the classic start menu
3) i start the classic start menu
4) issue disappears

5) i check "pre cache icons"
6) i exit the classic start menu
7) i start the classic start menu
8) the issue comes back!

restaring the computer doesn't solve the problem

Interesting, but not completely impossible. The code that pre-loads the icons is a bit different from the one that loads the icons when you open the menu.

I would suggest one of two things:
1) Leave the option unchecked. It may cause the icons to load with some delay the first time you open the menu
2) Try the latest beta version 3.9.0. The code to load icons is completely rewritten

I inistalled the beta :)
no problem with "pre-cache icons" now!

so, the cache is stored in %AppData%\CalssicShell\DataCache.db now, right?

Yes, version 3.9.0 does store the cache in a file. It stores more than just icons though, but also things that are expensive to retrieve, like targets for shortcuts, metro app information, and more.
