Windows Server 2012, upgraded from 4.0.6 to 4.2.6. After the installing 4.2.6 the entire start menu was missing!
Restarted the machine and it was back.
Surprised that a reboot was needed.

The Start Menu is launched after the installation completes from C:\Program Files\Classic Shell\ClassicStartMenu.exe. If your Explorer.exe process was hung and the DLL cannot be injected into it, the Start Menu may not work. Simply restarting Explorer.exe should do the trick.

It happened on two servers, and the explorer.exe process was not in a hung state.
Maybe the installation should be restarting explorer.exe?
