This is for "Classic Explorer Settings" on Windows 7 Pro:

When I add the map/Disconnect buttons on "Toolbar Buttons", one displays good (for map) and the other one (for disconnect) displays a plain-image icon (instead 3d-image icon) for the button.

And when I try to change the icon in "Edit item", with a "shell32.dll, 11" value, the preview shows the correct icon on the same dialogbox ( with title "Change Toolbar Button (DisconnectItem)" ), but out of there the icons doesn't change, not in "Current toolbar buttons" list, not on "Classic Explorer Bar".

The toolbar uses only the icons that are already found in Windows. Many commands don't have a suitable icon.
"shell32.dll,11" worked for me. Show me a screenshot of the settings when the icon is blank. Also export an XML file with your settings and attach it here.

OK, I send the files here, check that the icon for "Disconnect" is not the same of "shell32.dll, 11", it's similar, but not the same.

Too, some places the icon change is good (for example in "Edit Toolbar Button" Dialog Box or for a folder or direct access on the desktop), but not for the toolbar button of Classic Explorer....

Really, I don't understand why it doesn't show equal for each case... I hope files can help!!!

That is because the toolbar shows icon sizes of 24 x 24 by default. Open Classic Explorer settings, go to Toolbar Settings tab and change Large icon size to 32. Reopen the Explorer window.

An icon contains multiple images of different sizes (an sometimes different color formats). The images don't have to be exact scaled copies of each other. Often the small images like 16x16 are simplified compared to the large ones in order to make it look good.
